Public Register of NHS Foundation Trusts

Monitor maintains a Public Register of NHS Foundation Trusts. For each NHS Foundation Trust, the register contains (where applicable):

• a list of the members of the Board of Governors
• a list of the Directors
• a copy of the current terms of authorisation (including the current constitution)
• a copy of the latest annual report and accounts
• a copy of the latest forward planning document
• a copy of any notice given under Section 52 (failing NHS Foundation Trusts)

Information about what risk ratings Monitor has assigned to an NHS foundation trust is also included on the Public Register. Find out more about risk ratings.

The Public Register is listed below. Members of the public are welcome to inspect the register at our offices and copies or extracts of documents can be provided on payment of a reasonable fee to cover the cost of copying.
